Clinician Consensus on “Inappropriate” Presentations to the Emergency Department in the Better Data, Better Planning (BDBP) Census: A Cross-sectional Multi-center Study of Emergency Department Utilization in Ireland

Introduction:
Utilization of the Emergency Department (ED) for non-urgent care increases demand for services, therefore reducing avoidable attendance is an important area for intervention in the prevention of ED crowding. This study aims to develop a consensus among clinicians across care settings about the “appropriateness” of attendance at the ED in Ireland.
Method:The Better Data, Better Planning study was a multi-center, cross-sectional study investigating factors influencing ED utilization in Ireland. Following ethical approval, data was compiled in patient summary files which were assessed for measures of appropriateness by an academic General Practitioner (GP) and academic Emergency Medicine Consultant (EMC) National Panel. In cases where consensus was not reached charts were assessed by an Independent Review Panel (IRP). At each site all files were autonomously assessed by local GP-EMC panels.
Results:The National Panel determined that 11% (GP) to 38% (EMC) of n=306 lower acuity presentations could be treated by a GP within 24-48h (k=0.259; p<0.001) and that 18% (GP) to 35% (EMC) of attendances could be considered “inappropriate” (k=0.341; p<0.001). For attendances deemed “appropriate” the admission rate was 47% compared to 0% for “inappropriate” attendees. There was no consensus on 45% of charts (n=136). Subset analysis by the IRP determined that consensus for appropriate attendances ranged from 0-59% and for inappropriate attendances ranged from 0-29%. For the Local Panel review (n=306) consensus on appropriateness ranged from 40-76% across sites.
Conclusion:Multidisciplinary clinicians agree that “inappropriate” use of Irish EDs is an issue. However, obtaining consensus on appropriateness of attendance is challenging and there was a significant cohort of complex heterogeneous presentations where agreement could not be reached by clinicians in this study. This research again demonstrates the complexity of ED crowding, the introduction of evidence-based care pathways targeting avoidable presentations may serve to alleviate the problem in our EDs.

Maternal Expressed Emotion and Adjustment in Children with Epilepsy

Epilepsy in childhood may alter family relationships but the relevance of these changes for the increased rates of psychopathology has been little investigated. This study uses maternal expressed emotion (EE) to examine family relationships of children with epilepsy and the association with high risk for psychiatric disorder. EE was assessed using the Camberwell Family Interview carried out with the mothers of 22 schoolchildren with chronic epilepsy who were attending a general hospital outpatient clinic. Sixteen of these children had similarly aged healthy siblings who served as controls. High risk for psychiatric disorder in the children and mothers was assessed using behavioural, mood, and self-esteem questionnaires completed by mothers, teachers, and children. It was found that mothers showed significantly more emotional overinvolvement and a trend for more hostility towards their children with epilepsy than towards sibling controls. For the 22 children with epilepsy, maternal emotional overinvolvement was not associated with child behavioural deviance. High levels of criticism and, to a lesser extent, hostility did show associations with child behavioural deviance, and the strongest links were between maternal criticism and maternal rated antisocial and overactive behaviour in the child. Fewer positive comments by mothers towards the children were associated with child emotional symptoms and lower self-esteem in a number of areas. This study suggests that further research could consider the appropriateness of psychological intervention for families in which mothers are critical and hostile and whose children show antisocial behaviour.

The general ground for comedy is therefore a world in which man as subject or person has made himself completely master of everything that counts to him otherwise than the essential content of what he wills and accomplishes, a world whose purposes are therefore destroyed because of their unsubstantiality. Nothing can be done, for example, to help a democratic nation where the citizens are self-seeking, quarrelsome, frivolous, bumptious, without faith or knowledge, garrulous, boastful and ineffectual: such a nation destroys itself by its own folly.
Hegel is keen to distinguish the merely laughable from the comical in the sequel to this passage from page one thousand, one hundred and ninety-nine of the English translation of his Aesthetics. We may laugh at any contrast between subjective caprice and insubstantial action, while vice and evil are not in themselves comic: “There is also the laughter of derision, scorn, despair, etc. On the other hand, the comical as such implies an infinite light-heartedness and confidence felt by someone raised altogether above his own inner contradiction and not bitter or miserable in it at all; this is the bliss and ease of a man who, being sure of himself, can bear the frustrations of his aims and achievements.” (Is this condition of serenity, I wonder, attained by effort or by grace?) In comedy, “the ruling principle is the contingency and caprice of subjective life” whose nullity and self-destructive folly displays the abused actuality of substantial life. The aberration of the passions that rage in the human heart are drawn from “the aberrations of the democracy out of which the old faith and morals have vanished” (as Hegel describes Aristophanes's comedies). While in tragedy the powers which oppose each other as pathos in individuals are hostile, in comedy, “they are revealed directly as inwardly self-dissolving.” Comedy, as much as tragedy, is always divine comedy, “the Divine here in its community, as the substance and aim of human individuality, brought into existence as something concrete, summoned into action and put in movement.”
